The NYSPHCC has partnered with the United States Federation of Small Businesses in order to provide you with affordable group health insurance and many other important benefits.  The United States Federation of Small Businesses provides support, networking opportunities, group health insurance, and political lobbying for small businesses across the nation.  USFSB is an association of successful small business owners and independent contractors who are striving to grow their business, decrease costs, and improve the small business economy in their community.

In order to receive a health or dental insurance quote, a NYSPHCC member can take one of the following easy steps: search in the USFSB insurance clearinghouse by zip code or county to view the plans available, fill out a company census form provided in this mailing and fax it back to the NYC office of USFSB, or call the NYC office (718-701-0087) to speak to an insurance representative about your options.  When searching online, you can search by county or zip code and compare plans. You can then click “view details” at the side of each plan option in order to see a more detailed summary of benefits and click “choose plan” in order to download the forms needed with instructions about what is needed and where to send everything.
